Admission Requirements: - Applicants must be at least 18 years of age. - A government-issued photo identification is required. - Completion of a background check is mandatory. - Tuberculosis (TB) test results must be provided, dated within the past 90 days. -  Full payment for the training course is necessary. Students have the option to register for their course via the online registration form or in person. After submission, students are required to liaise with their designated Training Administrative Assistant, who will be assigned during the registration process, to confirm training dates. During this time, students will receive their enrollment kit, which contains essential materials. For any inquiries regarding admission, please contact the Student Services Office at the front desk. Withdrawal Policy

A student intending to withdraw from the program must inform the Program Manager of their decision and provide the reasons for their withdrawal.

Students enrolled in full-length occupational programs are encouraged to arrange an exit interview.
Should your response necessitate further attention, Akyaa Smith, the Program Manager, will be available to assist. ​

Attendance Policy

Student absenteeism is highly discouraged. Any approved absences and subsequent makeup training must adhere to regulatory standards, and makeup training will be conducted in accordance with the training schedule approved by the Program Manager. Purpose: To guarantee that students obtain all necessary training after an absence and that the makeup training is delivered in compliance with state regulatory standards. Scope: All training students Mandatory

Attendance: Students enrolled in the CNA Training Program are required to attend all scheduled classes, including both classroom instruction and clinical training. Failure to meet the minimum attendance requirement will result in disqualification from completing the program and being ineligible to sit for the Wisconsin Nurse Aide Competency Exam. The program manager has the right to approve or deny makeup instruction for any absences. If makeup instruction is granted, the maximum time allowed for makeup and the timeframe for its completion are outlined as follows: • Make-up sessions will involve attending additional clinical setting training as needed to fulfill the state’s 16-hour requirement. • It is the student’s responsibility to arrange make-up hours with their instructor.

Make Up Work Policy

The issuance of make-up work will be contingent upon the volume of work missed and will be managed by the Primary Instructor and Program Designee. Students are required to submit medical documentation or documentation for the cause of missed work.
• Students are allowed to make-up missed up time and will be assigned the training based on what was missed assigned by the primary instructor.
• Clinical hours can only be made up through primary instructor approval, and students are only up to one opportunity to make- up clinical hours.

Grading Policy and Course Structure

Course Structure
Course Total hrs. 91 hours 75 hours + 16 hours = 91hours The course consists of 75 hours of classroom/lab instruction, which includes the following: • Hartman Videos • Classroom lectures • Lab hours • Plus 16 hours of clinical experience Grading Policy 1. Written Quizzes:           o There will be 5 quizzes, each worth 10 points for a total of 50 points.           o To pass each quiz, a score of 80% or higher (8 out of 10 points) is required.           o Students must achieve a minimum of 40 points (80% of 50) across all quizzes.           o Retake Policy: Students are allowed one retake per quiz if they do not pass.
2. Final Written Exam:           o The final written exam is worth 30 points.           o Students must score 80% or higher (24 out of 30 points) to pass the exam.           o Retake Policy: Students have one opportunity to retake the final exam if they do not pass.
3. Skills Demonstration:           o Students must satisfactorily complete all required skills demonstrations as outlined in the skills checklist.           o A student who does not pass the skills portion will not be eligible for clinical participation.
4. Clinical Participation:           o Students must pass the final written exam and satisfactorily perform the required skills checklist before                participating in the clinical portion of the program.           o Clinical participation will be evaluated on pass/fail criteria based on performance and competency. Score a minimum of 40 points on the quizzes. 2. Score a minimum of 24 points on the final written exam. 3. Satisfactorily complete all required skills demonstrations. 4. Satisfactorily pass the clinical participation portion.

Drug & Alcohocol Policy

Five Star Educational Training Center, LLC apprehension regarding the possible negative impacts of alcohol and other substance use on the health and safety of students, in addition to their academic performance and the quality of patient care. It is anticipated that students will attend classes and clinical settings in a suitable mental and physical state that supports effective learning and ensures the delivery of safe patient care. To maintain the utmost standards within the nursing profession, it is imperative that students abstain from the use of illegal substances, alcohol, or any other drugs that may compromise their clinical judgment and performance. Additionally, health care facilities associated with the Five Star Training Educational Center mandate that students be free of drugs before commencing clinical experiences at their sites, thereby fostering a safe environment for patients, families, and If an instructor or staff member, students, clinical staff member, and or vendor of this training program suspects or observes the use of illegal drugs before, during, and any concerns which could protect the quality of life is required to report their concern to the Program Designee. If students face issues with substance abuse Five Star Training will provide resources for help and require the student to be free from any illegal drugs, and mental instability. Students can be required to undergo drug test at a clinical drug testing site, the student will be responsible for all cost. If the student appears to be under the influence and appears to be impaired. Five Star Training Educational will contact your emergency contact to only relate a concern to arrange for transportation. We cannot legally refrain the student from leaving our premises. We will make every effort to arrange transportation if needed. If students are sent for drug testing, you must report within one hour from the notice in writing from program designee and may not return until a proof of negative testing. At no point does Five Star Educational Training Center, LLC assume the responsibility of costs associated with testing, or transportation. If you fail drug testing, by the end of the business you will be withdrawn from the program, and not allowed to reenter Five Star Training Educational Training, LLC. No refunds will be provided.

Refund Policy

•A full refund for withdrawal requests made 10 days before the program start date. • Refunds processed within 30 business days of request. Should a student choose to withdraw from or terminate the course, they will not receive a refund for any fees incurred. Program Manager will consider refund appeals in certain circumstances, such as illness, administrative errors, or military transfers. There will be No Refunds of tuition if you are dismissed from the program for violating our student conduct policy.
